Dartmouth Health has exclusively retained Kirby Bates Associates to lead a national search for a System Director, Care Coordination to lead system wide Care Coordination functions (Capacity Coordination, Social Work, Navigation, and Case Management), across the health system and the health plan.

Dartmouth Health is seeking a dynamic and innovative System Director, Care Coordination, to lead system-wide integration of care coordination. This newly created role is a unique opportunity to shape the future of patient care and drive significant improvements across Dartmouth Health.

Reporting to the System VP, Care Coordination, the System Director, Care Coordination will work closely with the System Director, Utilization Review and hospital CNOs to develop and execute the care coordination strategic plan for the system.

Qualifications:

  • Bachelor’s and Master’s degree required, one of which must be in Nursing
  • A minimum of 5 years of leadership experience in care coordination/case management in an integrated healthcare delivery system.
  • Certified as an Accredited Case Manager, Certified Case Manager, or Case Management Administrator required or obtained within six months.
