Kaleida Health has exclusively retained Kirby Bates Associates to identify their next VP Care Management, Post-Acute Care Transition (VP) to provide executive leadership for Care Management (CM), Discharge Planning, and Post Acute Care Transitions across the continuum.

The VP will have responsibility across Kaleida Health System which includes Buffalo General Medical Center/Gates Vascular Institute, DeGraff Medical Park, Millard Fillmore Suburban Hospital, John R. Oishei Children’s Hospital, Bradford Regional Medical Center and Olean General Hospital, as well as Great Lakes Integrated Network, HighPointe and DeGraff skilled nursing facilities, associated partners across the system, and community organizations.
Kaleida Health also offers home health care through the Visiting Nursing Association of WNY, Inc. In addition, Kaleida Health’s hospitals support residency training programs of the University at Buffalo Jacobs School of Medicine and Biomedical Sciences, training hundreds of residents each year.
Job Description:
The VP role supports the System Chief Nurse Executive (CNE) in providing executive leadership and guidance for critical areas of Care Management (CM), Discharge Planning and Post-Acute Care Transitions. This work includes the integration of Acute Care Managers, Skilled Nursing Facility screeners, Care Coordination, Discharge Planners, Outpatient Adult Care Management, and Outpatient Pediatric Care Management. In addition to daily oversight of operations, the VP will partner with Utilization Review, Revenue Cycle, and Clinical leadership to create patient/family centric processes and interdisciplinary relationships that result in seamless patient care and will be responsible for implementation of successful solutions that are evidenced-informed and measurable. The VP will be responsible for the system’s financial performance related to care progression, denial prevention, and efficient patient throughput and for providing consultative, leadership, educational development, and support services to the acute care facilities and affiliated networks in areas of CM, discharge planning, clinical management, and improved operational efficiencies.

Qualifications:
- Bachelor’s degree in nursing required, Master’s degree in Health or Business preferred.
- 10 years of care management/utilization management experience in a hospital or ambulatory setting.
- 7 years of progressive leadership experience.
- Case Management Certification (ACM, CCM, HCQM, CMAC) preferred.
- Leadership experience within a union environment preferred.
