Vice President Care Management, Post-Acute Care Transition

Kaleida Health has exclusively retained Kirby Bates Associates to identify their new VP Care Management, Post-Acute Care Transition (VP) to provide executive leadership for Care Management (CM), Discharge Planning, and Post Acute Care Transitions across the continuum.

As the largest healthcare provider in Western New York, Kaleida Health serves the area’s eight counties with state-of-the-art technology and comprehensive healthcare services.

The VP will have responsibility across Kaleida Health System which includes Buffalo General Medical Center/Gates Vascular InstituteDeGraff Medical ParkMillard Fillmore Suburban HospitalJohn R. Oishei Children’s Hospital, Bradford Regional Medical Center and Olean General Hospital, as well as Great Lakes Integrated Network, HighPointe and DeGraff skilled nursing facilities, associated partners across the system, and community organizations.

Kaleida Health also offers home health care through the Visiting Nursing Association of WNY, Inc. In addition, Kaleida Health’s hospitals support residency training programs of the University at Buffalo Jacobs School of Medicine and Biomedical Sciences, training hundreds of residents each year.

  • Partner with Utilization Review, Revenue Cycle and Clinical leadership to develop interdisciplinary relationships resulting in seamless, patient /family-centric care.
  • Official healthcare partner of the Buffalo Bills.
  • Salary target $171,000-$200,000 plus bonus.

 Qualifications:

  • Bachelor’s degree in nursing required, Master’s degree in Health or Business preferred.
  • 10 years of care management/utilization management experience in a hospital or ambulatory setting.
  • 7 years of progressive leadership experience.
  • Case Management Certification (ACM, CCM, HCQM, CMAC) preferred.
  • Leadership experience within a union environment preferred.
